Condition care

Support for the conditions families worry about

Practical, person-first care at home for Barking and Southampton.

We care for the person first, and the condition second. We are a home care service: we work alongside GPs, nurses and therapists and follow their guidance. We do not replace clinical care.

✨

All-Inclusive Senior Care

For people who need a little help in many areas, we join up the pieces so nothing is left to chance.

A question we may ask: What does a good day look like, and what would you most like to keep doing?

How we can help

  • One plan covering personal care, meals, company and home
  • A single point of contact for the family
  • Regular reviews as needs shift
  • Encouragement to keep doing what the person can do for themselves
💪

Stroke Care

After a stroke, small daily routines matter. We support recovery at the person’s pace and follow the guidance of their health team.

A question we may ask: What exercises or guidance has the stroke team given us to follow?

How we can help

  • Help with moving, dressing and eating in line with therapist advice
  • Patience with speech and communication; time to answer, no finishing sentences
  • Prompting practice exercises agreed with the care team
  • Watching for changes in mood or confidence and sharing them with family
  • Making the home easier and safer to move around
🌺

Elderly Care

Growing older at home is easier with a dependable helping hand and someone to talk to.

A question we may ask: Which parts of the day feel hardest right now?

How we can help

  • Help with washing, dressing and meals
  • Medication reminders as set out in the plan
  • Company, walks and outings
  • Home safety awareness and routines that reduce falls
  • Support that grows only as far as the person wants

Cancer Care

Treatment can be tiring. Practical support at home lets people save energy for what matters.

A question we may ask: Which days are hardest, and what would make them easier?

How we can help

  • Help on and around treatment days, including getting to appointments
  • Gentle meals and drinks suited to appetite and energy
  • Rest-friendly routines and flexible visit times
  • Kind company and room to talk, or not to talk
  • Giving family carers a break

Complex Care Conditions

When several conditions overlap, clear communication and careful planning keep everyone on the same page.

A question we may ask: Who else is involved in care, and how would you like us to work with them?

How we can help

  • A written plan that sets out each need and who does what
  • Joined-up working with GPs, nurses and therapists as agreed
  • Consistent carers who know the person well
  • Clear records and prompt updates to family
  • Honest conversations if a need is beyond what we can safely provide

Parkinson's Care

Parkinson’s varies from hour to hour. We plan around the person’s best times and never hurry.

A question we may ask: When are the best and hardest times of day, and what helps most then?

How we can help

  • Medication reminders at the times the prescriber has set
  • Unhurried help with dressing, buttons and meals
  • Support with safe walking and transfers
  • Time for speech and movement; no pressure on slow moments
  • Flexible visits that follow good and difficult periods
